You have most probably seen such notes in the parish books - cause of death and then a number of a death survey card.

Structure of Records

Kate Challis is preparing headlines of columns, so I decided to look at their contents and abstract the common patterns that may help you to with basic orientation.
More examples explained in Czech are also on Blanka's site. So there are not mentioned Czech records, but their structure is the same and could by easily abstracted by analogy from the Latin or German ones.

Death notes

You probably know them. Short notes in "cause of death" column in death registries. There is often written something like "Úmrtní list z Radlic č. 21 dne 15. září 1873", "úmrtní ohledací lístek č. 15 dto 28. Januar 1836" or just a simple "č. 21". What do these notes mean? What they looked like? And can you still find them somewhere?

Death causes dictionary

As I was writing a blog post about epidemies, I was already thinking about creating basic dictionary of death causes - Czech-German-English dictionary, because many of parishes in Bohemia, Moravia and Silesia had parish books in German. And here it is. These are just those most common death causes as taken from parish books of Horní Záhoří (South Bohemia), Slabce (Central Bohemia), Nový Přerov (Moravia) and Kateřinky (Silesia).
